Job Summary:
The Behavioral Health Case Manager II provides case management and care coordination for adults with serious mental illness, including those with co-occurring substance use disorders. The role supports individuals with functional limitations through assessment, individualized service planning, coordination of behavioral health and recovery-oriented services, advocacy, and collaboration with multidisciplinary teams and community partners. Responsibilities include promoting symptom self-management, supporting medication adherence, and assisting with activities of daily living (ADLs) to help clients achieve stability, independence, and overall well-being.
Essential Functions:
1. Interviews clients, family members or any other persons deemed necessary to obtain the most complete case history possible.
2. Provides guidance and relevant information to the client's family to enable them to facilitate solutions and promote positive interactions among family members.
3. Evaluates information derived from client interviews, case history, and other pertinent sources and refers to proper channels for review.
4. Prepares narrative, statistical reports and court reports as necessary.
5. Provides input as a member of a multi-disciplinary team in the assessment of client human service needs and treatment planning, enabling the most beneficial placement in nursing home, drug/alcohol rehabilitation or other special care facility.
6. Prepares necessary forms of application for services or benefits. Contacts appropriate social agencies and coordinates referrals.
7. Appears at court proceedings as required testifying to the social and environmental issues affecting the client.
8. Provides technical guidance to organizations and staff regarding findings related to client care or provides consultation to other caregiving agencies.
9. Provides consultative services to state and private agencies; may participate in community projects.
10. Advocates for clients and families in the protection of rights; obtains consent for treatment.
11. Uses automated information systems and computer software to research, review, evaluate, and report on health/human service data.
12. Communicates and interacts effectively with people across cultures, ranges of ability, genders, ethnicities, ages, and races.
Minimum Qualifications:
1. Possession of a Bachelor's degree or higher in Behavioral or Social Science or related field.
2. Six months' experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.
3. Six months' experience in providing clinical treatment, counseling or determining eligibility for services or benefits as part of a treatment team.
4. Six months' experience in interpreting laws, rules, regulations, standards, policies, and procedures.
5. Six months' experience in narrative report writing.
6. Valid Driver’s License.
